How to correct wrong NSO birth certificate detail errors

Ever experienced wrong NSO birth certificate information / details (errors)? wrong spelling, wrong gender or sex, typographical errors, clerical errors, wrong date of birth, misspellings, wrong place of birth, mother’s or father’s name, no middle name and more discrepancies? Court Decree (Correction of Entries in Civil Registry Records):

(1) Certified copy of the Court Decree of Correction of Entry/Change for First Name/Cancellation of Entry
(2) Certificate of Finality
(3) Certificate of Registration of the Court Decree
(4) Certified Machine Copy/Photocopy of Annotated Birth/Marriage/Death Certificate or Certified True Copy (CTC) of Birth/Marriage/Death Certificate with the appropriate annotation

    hi ask q lng po kung paanu magpacancell ng entry s birthcertificate.. di po married ang parents pero nakalagay po ay married..pede po bang ipacancell yon? inaacknowledge naman po ng tatay ng bata na gamitin surname niya kaya gamit din po surname ng tatay. anu po dapat gawin? thank you po and godbless
