NSO Birth Certificate Change Name

There are so many issues involved if you need your birth certificate change name, change middle name, date of birth, change surname and even the sex or gender. Many Filipinos have this problem (wrong spelling) with their certificate of live birth records in the National Statistics Office (NSO) or Census.

If you are one of them, please make sure that you go directly to the NSO website and refer to the rules on how to correct your birth certificate name. Be sure also that you will not go with fixers and beware of them. There are plenty around. sir/mam problema ko po ung birth certificate ko.mali kc ung middle initial ko.instead RAGOS ang nklagay RAGSA..pno po b ggwin ko pra mkkuha n ko ng pasport.pwd po bng dyan mismo s NSO ko aayusin ang birthcertificate ko o need ko p pumunta s province kng sn ako pnnganak..kc un din po isng problem ko wla po ko time pumunta dun s province..wait ko nlng po ang reply nyo…thanks!!!!

Hi, I need your assistance regarding my surname.
I’ve been using my father’s family name since birth, baptismal, school , work, voter’s registration until marriage. Everytime I get a copy of my BC sa census laging no records kaya I always use affidavit. I wanted to get a passport, so I tried to file a late registration, doon sa birthplace ko, and later I found out registered pala ako under my mother’s family name. My parents are not married and they are both dead now.
I would be very glad if you can recommend me a lawyer to work on my problem.

ask ko lang po kung paano ma-check online yung record ng nanay ko bago po ako magrequest ng copy sa nso. sayang naman yung bayad kung magre-request na agad ako tapos wala pala sa records ng nso ang name ng nanay ko.

Hi ask ko lang po.. pano po ung procedure para po matanggal ung second name ko. halimbawa po ang name ko po ay John Mark E. Cruz, gusto ko po patanggal ung John. at saan ko po ito gagawain para matanggal po. nawa’y masagot niyo po ito.
