NSO Court Decree and Correction of Entries Requirements
Correction of NSO documents entries require you to give certain papers to submit to the NSO or National Statistics Office. Below is a list of NSO court decree and correction of entries requirements. NSO Certificate of No Marriage (CENOMAR) Delivery
Court Decree (Correction of Entries in Civil Registry Records):
(1) Certified copy of the Court Decree of Correction of Entry/Change for First Name/Cancellation of Entry
(2) Certificate of Finality
(3) Certificate of Registration of the Court Decree
(4) Certified Machine Copy/Photocopy of Annotated Birth/Marriage/Death Certificate or Certified True Copy (CTC) of Birth/Marriage/Death Certificate with the appropriate annotation

my birth certificate have double entry. Both are late registered. I register my birthdate as September 29, 1963 and later I have registered my birthdate as September 29, 1959 which I am using already for the past 33 years. Three years ago, the previous entry came out and the lawyer told me that is very complicated to fix. I am really dissapointed. What I am going to do. Plas.help me.
good day po mam, ask lang po ako kung paano ko po i remove ang apelyido ng anak ko gamit ang apelyido ng asawa ko. Gusto ko pong palitan sa apelyido ko at hindi na ang apelyido ng asawa ko. Magkano po ang mababayaran ko. at ano po ang mga papeles na kakailanganin po. kasi kailangan nya ng kumuha ng passport. Sana po matutulongan nyo po ako sa problema ko.
