   yung lumang birth certificate ko ang complete name ko ay
   surname 1st name midle initial
   CRUZ – MEL RAYMUND – FESARET

   yan po ang gamit ko mula pa ng bata pa ako hanggang college

   nung nag request ako sa NSO iba na ang lumabas na midle initial ko
   sur name 1st name midle initial
   CRUZ – MEL RAYMUND – FERNANDEZ

   medyo na guguluhan ako kung paano nangyari tong case na to
   ang record ko tuloy sa SSS ko ay MEL RAYMUND FESARET CRUZ
   sa city hall ng caloocan ako kumuha ng birth certificate ko
   para sa SSS

    Hello Tess and the others, Malalaman lang po kung kasal ang isang tao kapag nagpakuha kayo ng NSO CENOMAR (certificate of no marriage) pero kakailanganin ninyo ng mga documents to present (such as valid ID) from the person na kukuhanan ng CENOMAR and request letter na pirmado nya (attesting na pumapayag sya magpakuha ng nasabing NSO CENOMAR).

    Dto rin po ba sa NSO nag re-request ng “Family Tree”. Un pong mate-trace ung relationship ng pamilya sa isa’t isa. Gagamitin ko po ito to show the relationship between siblings. My sister who is married to another nationality wants to sponsor me so I can go to that country and they are requiring for this proof that we are indeed sisters.

    kumuha ako sa NSO noong Februry 2010 ng birthcertificate. pinanganak ako ng 1938, Nag file po kase ako ng Visa for japan, Ang sabi po ng Embassy, hindi daw clear ang birthcertificate na galing sa NSO, kailangan ko daw kumuha ng bagong certificate doon mismo sa probinsiyang pinang galingan ko, Napakalayo po ng Catbalogan Samar, 50 yrs .na akong hindi nakakapunta doon mag ba barko pa po, ilang araw den ang papunta doon, Ano po ba ang dapat kong gawin para matanggap ng japan embassy ang kinuha kong Birthcertificate na galing sa NSO.

During the seminar, nasabi po ninyo na as long as we finish our outputs you will give us the said honorarium pero matatapos na naman po ang June at hindi pa po namin nakukuha ang pera na dapat sa amin, na aming pinaghirapan.      I just got married last April 2010, then pina reciv po yong marriage certificate namin sa city hall 2 weeks after the wedding. I’m currently working abroad and I want to petition my wife immediately, but however ang requirement to get her is to have the marriage certificate copy from NSO. Unfortunately, we will get it after 6 months to 1 year pa daw.

     We tried to get endorsement letter from city hall registrar’s office for us to process na lang to NSO (as suggested by others), but they had informed us that our marriage certificate was already forwarded to NSO and they cannot do anything.

     What would be the alternative procedure to get our married certificate ASAP at NSO?
     Please advice.

     My uncle who is in the US request me to acquire his NSO birth cert. He was born 1947 in Magsingal, Ilocos Sur but later found from his birth cert shows 2 names. When I informed him, he confidently believed that he never used the other since he started schooling & all his documents bear only the name he is currently using. My grandparents were already dead & I believe no one will testify for him except my mom & his cousins, since his aunts/uncles were already abroad.

     We need to be enlightened on this matter.

     Please guide us on the right & correct procedure. I only have his 3 documents (baptismal, elementary diploma & college certificate showing only 1 name that he is currently using) with me. I was also informed that we will be looking at around Php 6000 charge.

     What do I advice my uncle?

     I’m a single mom, when I was filing my son’s birth certificate at the Civil Registry of Manila the receiving officer deleted my middle name on it, when I asked the reason why she deleted it she said that there was a memo that illegitimate should be registered without a middle name. Since I cannot do anything about it as it appears to be the policy I let her register my son’s birth certificate without my middle name on it.I have been researching since then if a civil registrar has an authority of refusing to register an illegitimate child with the middle name of the mother or authority to delete it, when I came across of an article of the Phil. Star dated July 16, 2010 where you told reporters in a forum held at the Hotel Rembrandt in Quezon City that the civil registry manual provides that the registration of the middle initial or name is optional.
     I am seeking your good advice as an authority on this matter on how can my child use my middle name to eliminate the stigma of illegitimacy and for the best interest of the child.

     i have a one letter typographical error in my birth certificate (surname), what kind of process para ma pa change that one letter correction, & how long it takes for the processing.

     Good day! just want to know how long is the procedure for correcting one letter in your family surname on the Birth Certificate. As I read RA 9048, i can’t find the 3 months processing time that the municipal staff told my father when he submitted his docs to the municipality. Kindly inform me if this is true, if so, is there other legal options available to us like affidavit from a lawyer just to expedite the processing since time is of the essence.      I start to work in abroad since 2003..,now im here in the phil’s because my contract in Taiwan is finish and for my chance to get together with my family for vacation also.      I start to get all the originals file’s of my Parents,bro, and sister to use this file to certified there is human error..my name i use FREDERICK Talastas. ORBETA but in my NSO is FREDERICK Talastas, ORBITA ..means i have a problem w/ my surname because of the letter ( i )..then I’ll go to CITY Hall of manila for the correction i really get mad because of the processing need is 3-4mons…im very dis-appointed w/ our phil.gov’t..time i was there im begging to the staff help to do something for the correction..but i was failed…

     All i know is the PHIL..GOV”T is there to all people who needs help…But where i am now ( yong dating ofw bagong bayani ng pilipinas ) Gov’t din magpapabagsak sa pangarap ng mga anak ko makatapos ng pag-aaral…

     I have a problem getting my birth certificate. I requested one from your office but you do not have record of my birth. I went to my place of birth, Aparri, Cagayan, and requested from the Office of the Municipal Registrar and this is the report:

     We certify that the records of BIRTHS filed in the archives of this office include those which were registered from October 1945 to present. However, the records of births during the period July 1945 to September 1945 were totally destroyed by World War II, hence, we cannot issue as requested a true transcription from the REGISTER OF BIRTHS or true copy of the Certificate of Live Birth of Faustino G. Valera who is alleged to have been born on July 01, 1945 in this municipality of parents FAUSTINO L. VALERA and ESPERANZA GALLARDO.
     We also certify that for every registered birth, this office submits a copy o the Certificate of Live Birth to the Office of the Civil Registrar General, National Statistics Office. In view of this, the interested party is hereby advised to make further verification in the archives of that office.

     This was signed by the Municipal Civil Registrar and his Admin. Aide.
     What will I do to get a Birth Certificate from your office. All documents in my behalf show the above birth information.

     Noong kumuha po ako ng birth Certificate ko sa LCR at NSO ay nalaman ko na ang name na nakasaad dito ay magkaiba sa name na ginagamit ko mula pagkabata at sa lahat ng mga credentials ko. Kailanman ay hindi ko ginamit ang name ko na nakasaad sa Birth Certificate ko. Kailangan ko po kasi na magsubmit ng NSO Birth Certificate na naksang-ayon sa name na ginagamit ko.
     Nais ko pong malaman kung paano ko po i-process ito at ano po ang procedures and requirements ang kakailanganin ko para palitan ang name ko na naka register sa NSO at sa Civil Registrar.

     I am asking your good office for help for the NSO Birth Certificate Late Registry regarding with my mother. NSO Blocked my mothers 1963 LATE REGISTRY because unfortunately an older 1961 Birth Certificate has been found. We have been using her LATE REGISTRY 1963 for almost 20 Years and why did only now these 1961 Birth Certificate popped out when all Documents have been following the 1963 LATE REGISTRY. If this have been found earlier these problems about documents not tallying would have been repaired.

     We already filed a case in court and we were denied a grant to use her 1963 Late Registry.

     My Grandmother cannot read or write and also suffers from Alzheimer disease and just recently passed away. My mother only believed on my Grandmothers and Grandfathers statement about her Birth year and when she went to NSO to look for a Birth Certificate 20 years ago there were no found 1961 Birth Certificate so she filled a Late Registry 1963 year of birth. When she went to NSO these year to get a Late Registry they denied us. They are now showing us a Birth certificate year of birth 1961 that has a supplemental report of corrected name from baby girl to my mothers name. My mother has an older sister that already passed away and her birth certificate was younger than my mother. My aunt has lots of birth certificate that doesn’t correspond to my mothers birth year 1963…

     We really need the 1963 Late Registry even just a copy. Our Japanese Visa is in great jeopardy if we change my mothers birth certificate here. What would they say. Will they understand our systems in NSO. Would they accept court orders to undo the filed 1963 late registry that it is not my mothers fault without consequences of denying our visa. If we follow the 1961 Birth certificate what would happen to all of our document already followed the 1963 Late registry. We have been using the Late registry for almost 20 years and NSO just decided to block it without hearing our side even the court. NSO has a fault too why only now this 1961 Birth certificate found. Did they not check their system when there are other Birth certificate before granting a Late registry. How do we file for an appeal? Is it possible to put another supplemental report on the 1961 Birth certificate that it was the 1963 Late registry. Could you help me obtain a Late Registry 1963 of my mother. We are extracting every possible options.

My marriage was annuled August of last year at nasubmit lahat ng documents ko sa NSO Manila noong 3rd week of January this year gaano ba kahirap makuha ang papeles na ito at until now hindi pa rin nalalagay sa data base at pauli ulit pang binabalik sa gayong kumpleto na ang requirements ko dahil yung nagsubmit mismo ay taga NSO sa aming probinsya.      Good day! How would someone know if his or her birth certificate were registered in NSO or Philippine Embassy? For example, a person`s birthplace was outside the Philippines. A person was issued a passport using the foreign country`s Birth Certificate.

     Good Day! Pls help us to fixed our problem.what the best thing to do..My father has no copy of his original Birth Certificate for many years, we tried to get the copy in NSO main office but unfortunately he had no records at all.the staffs advise us just enroll my father as late registration but we will not doing that. it would be resulting to conflict of his records coz he had provided his old copy of his birth certificate which has gone and not able to retrieve which his submitted to his entire company when he was working, and now hes 59 years old he should avail his retirement but the problem is his Birth Certificate documents. he’s from surigao its hard for him to travel to go there just to arranged and find his records which where exactly he had his old birth certificate copy nor havn’t any news also to our relatives that we supposed to ask to help us to inquire for my father records.
