Up to now I have not been able to remit my Philhealth monthly contribution for the simple reason that we could not secure remittance forms here in Toledo City, Cebu. One bank, Green Bank has forms but the guard refuses to give me for the reason that I am not an account holder of the said bank. I would like to request that this form be made available to its members from remitting banks, preferably PNB or RCBC. For paying the monthly remittance of 100 pesos,if we secure forms in Cebu City, which is about 60 km, we have to spend more than 200 pesos.

It would be an advantage for the members as well for Philhealth if you make it easier for the members to remit their monthly contribution to increase the funds of the agency.

Kindly reply through my e-mail ad for any development.

Thanks and more power to you
