    hi ask ko lng po if magmemember din po ako ahm anu po ung mga requirements…atsaka pwede ko po ba maging dependent o benificiary ang mama ko khit mgkaiba kmi ng apelyido,ang pangalan nya po kasi is maritess soliman espenilla at ako naman julie ann cano soliman pwede po ba yun?o kung sino yung nasa birthcertificate yun yung susundin?

    My aunt passed away more than six (6) months ago. She was a government teacher (retired). Before she died, she has an undeposited check from Philhealth worth P1,800.00 dated last April, 2010. She has 2 siblings, both are not capable of travelling. As per her Philhealth data, she did not designate any dependent. what is the procedure in the replacement of check and who may process and follow-up the replacement of the check? Thanks!
