B-304 SSS Form EC Claim for Temporary Total Disability or Sickness
Here is the free download for your sickness benefit claim. This is the B- 304 SSS form which is used for the “EC Claim for Temporary Total Disability or Sickness”. You may give this to the Employees Compensation Commission (ECC) to be able to claim your sickness benefit depending on the hospital and doctor’s recommendation.
This form is also used as a requirement by hospitals before you can claim for your EC sickness benefit as well as temporary or total disability being a contributing and qualified SSS member of the Social Security System Philippines. The download link is found below.

I just want to ask what should i gonna do to enroll again to the sss website before i already register but unfortunately by id was been blocked so i didn’t allow the sss website to resgistered again
i am a sss member before but i lost my wallet together with my sss id and number,until not i did not pay anymore my monthly contribution coz i dont know the number…can i ask a copy of my id number ..thank you

You can only retrieve your SSS ID number via the nearest SSS branch.
about my ID i like to send it here in cebu SSS that was captured last Sept.1 2008 and my add.CANTILA POB.OCC.CONSOLACION CEBU.
