How to apply for a new SSS ID: Requirements
Here is short guide on Social Security System requirements and how to apply for a new SSS ID. This is either if you lost it or just on your way to a 1-year contribution of your monthly premium and is already qualified to apply for a new ID.
SSS Branches - Philippine Social Security System
For New ID:
All you need is your duly filled up and accomplished SSS Form E-1 (Personal Record). Submit it together with the following documents
original/certified true copy and photocopy of any of the following:
- baptismal
- birth certificate
- driver’s license
- passport
- Professional Regulation Commission (PRC) Card or Seaman’s Book
If you don’t have any of these documents, any two of the following, one IDs listed below of which with recent photo and date of birth, are acceptable.
- Alien Certificate of Registration
- ATM card with the cardholder’s name
- ATM card and certification from the issuing bank that the account number belongs to the cardholder, if the card does not bear the cardholder’s name.
- Bank Account Passbook
- Birth/Baptismal Certificate of children
- Certificate form:
- -Office of the Southern/Northern Cultural Communities
- -Office of Muslim Affairs
- Certificate of Naturalization issued by the Bureau of Immigration
- Company ID
- Company Representative Authorization Card issued by SSS
- Credit Card
- Fishermen’s Card issued by Bureau of Fisheries & Aquatic Resources (BFAR)
- GSIS Card
- GSIS Certificate of Membership
- Health or Medical Card
- ID Card issued by Local Government Units (e.g. Barangay/ Municipality/City)
- ID Card issued by professional associations recognized by PRC
- Life Insurance Policy
- Marriage Contract
- NBI Clearance
- Overseas Workers Welfare Administration (OWWA) Card
- Pag-Ibig Member’s Data Form
- Permit to Carry Firearms issued by the Firearms & Explosives Unit of the Philippine National Police (PNP)
- PHIC Member’s Data Record
- Police Clearance
- Postal ID Card
- School ID
- Senior Citizen Card
- Tax Identification Card/Affidavit
- Transcript of School Records
- Voter’s Identification Card/Affidavit
However, surviving spouses and guardians aged 60 and above securing an SS number for the Annual Confirmation of Pensioners (ACOP) program, may be allowed to submit secondary documents with recent photo and age as stated above. (source: sss.gov.ph)
For Lost SSS ID:
Secure an affidavit of loss duly signed and notarized by an attorney at law or lawyer. You will also need at least 2 valid IDs for confirmation of your identity.

To whom this may concern,
I have been a member of sss since 1980″s.And I have already an old ID.In fact I have completed the number of contributions required in order to receive pension,but I decide to still continue giving my contributions till now.
.
My concern is that, I have applied my new ID for 3 times already ( with digital photo needed as it was done in sss branch Butuan City)and the last application was last year.I have series of applications bec.it is always at a lost since it never came to my post everytime.It is my husband always following it up bec.I can be home only once a year from abroad.How to do about it?Do I have to pursue it? and submit the above requirements? If ever same thing happens,Does my old ID without picture ,enough in case I will be receiving my pension soon?

I already have an SSS number and have completed my contribution. However I could not retrieve my old original ID card.
Should I have to accomplish an Affidavit of Loss to get the new SSS ID card? Where do I process the application?

You will need po the death certificate unang una… ipresent nyo po ito sa nearest SSS branch and from there tanong nyo na po kung ano pa ang kailangan.
where can i get a new sss id.. is that important that i need to come in main sss?
It seems na only the main SSS had the ID capture machine for SSS ID application so it is best to go there if you don’t want to end up going to branches where the machines are either long broken or not running.
