How to Get Seaman Book or SIRB in the Philippines

There are actually 2 ways you can get a seaman book in the Philippines. One is to apply in person to the MARINA office, the issuing government agency for seafarers looking to get a copy of the Seafarer’s Identification and Record Book. The other way is now made easier as you won’t have to fall in the very long line and sleep outside. This new way is called the seaman book application thru online appointment system. This procedure is similar to how a DFA passport applicant applies for this travel document.

Please see if you have the complete documents and if you do pre-qualify to apply. Make sure to appear in person on the date you opted for a schedule of appointment. It will be void on no-show up. Furthermore, online appointment doesn’t mean no appearance. It only means you can schedule an application and prepare for all you need prior to acquiring a copy of the seaman book. You will be required to appear for the purpose of image capturing and signature signing… etc. In the meantime, you may download a copy of the application form in applying for SIRB here.

Steps to Schedule for Seaman Book Online Appointment System Reservation

  1. Visit the official website for scheduling an appointment here
  2. When on the website, select SIRB as the document to apply for
  3. Choose your desired appointment date and time to report
  4. Fill up the online application form then download a copy of it while making sure there is an appointment control number and scheduled date and time to apply
  5. Prior to your schedule to report at the listed office below, make sure you have all the original and photocopied documentary requirements. You will not be entertained if you bring incomplete papers
  6. Proceed to the MARINA SCDD office at the 5th Level SM City Manila, Concepcion St. corner Arroceros St., Manila for seaman book document application. Other documents like COP and Licensure is to be processed in STCW processing center
  7. Report at least 30 minutes before your scheduled time and date for preparation allowance. Note that late comers will not be entertained
  8. Show your printed application form to the appointment counter for verification
  9. Get your queue number then wait for your turn to be called. Your number will appear in the monitor where a corresponding window number appears where you will bring your documents
  10. After passing the evaluation, pay the SIRB processing fees to the Cashier Department
  11. After payment, you will be asked to proceed to image capturing window where your photo will be taken
  12. Choose a mode of claim option: 1.)MARINA office or 2.)Opt in for a door-to-door delivery
  13. If your choice is #2, please pay at the delivery counter the document delivery fees.
  14. For mistaken multiple appointments, only the last valid and completed online appointment reservation will be counted

Seaman Book Application Requirements

Documents for New:

  1. Birth Certificate on Security Paper (SECPA) or Passport
  2. Valid NBI Clearance (visa seaman)
  3. Valid Basic Safety Training (BST) certificate
  4. Official Receipt of Basic Safety Training (BST) Course

Documents for Renewal:

  1. Original SIRB
  2. Valid NBI Clearance (visa seaman)
  3. Basic Safety Training (BST) Certificate

Re-Issuance of SIRB for Ship Officers:

  1. Original SIRB
  2. Valid NBI Clearance (visa seaman)
  3. Basic Safety Training (BST) Certificate
  4. PRC License or Board Certificate
  5. STCW, 1978/1995 Certificate
  6. PRC I.D. for Licensed Officers

For Lost or Damaged SIRB: (regular seaman)

  1. Valid NBI Clearance (visa seaman)
  2. Valid Basic Safety Training (BST) Certificate
  3. Duly Notarized Affidavit of Loss or Damage

For Lost or Damaged SIRB: (officer seaman)

  1. Valid NBI Clearance (visa seaman)
  2. Valid Basic Safety Training (BST) Certificate
  3. Duly Notarized Affidavit of Loss or Damage
  4. PRC License or Board Certificate
  5. PRC I.D. for Licensed Officers
  6. STCW 1978/1995 Certificate

For Onboard Application:

  1. Letter request from the Company/Agency
  2. Approved employment contract
  3. Crew list duly signed by the Master
  4. Clearance from the MLAO as to monetary obligation
  5. Affidavit by the seafarer and confirmation by the Master
  6. Latest passport size photograph
  7. Photocopy of the following

A. For Regular Seafarers:

  • Photocopy of SIRB:
  • Valid Basic Safety Training (BST) Certificate

B. For Ship Officers:

  • Photocopy of SIRB
  • Valid Basic Safety Training (BST) Certificate
  • PRC License of Board Certificate
  • STCW, 1978/1995 Certificate
  • PRC I.D for Licensed Officers
